Hours:                S.A.G.E. assessments are available (12:00 p.m. - 4:00 p.m. Monday; 8:45 a.m. - 12:00 p.m. Friday) and are a minimum of three hours in duration

Fees:                  Covered by Medicare and supplemental Insurance

Eligibility:           Individuals age 60 or older requiring such services

Services:  

·      Consultation by a team of geriatric specialists including: gerontologists, a nurse practitioner, dietitian, and social worker

·      Services for persons with complex medical, nursing, nutritional, social, and emotional needs

·      Home visits by a nurse practitioner are available for persons in the Columbia area

·      Multiple issues that may concern older persons and their families can be addressed, such as:

o     Physical limitations

o     Cognitive, emotional, and behavioral changes

o     Difficulties with independent living

o     Alternate living arrangements

o     Evaluation of medications

o     End-of-life issues
